What is NCCI?
The National Correct Coding Initiative (NCCI) was implemented by CMS to prevent improper payment of codes that should not be billed together. NCCI edits define pairs of codes that are considered mutually exclusive or represent services bundled into a more comprehensive procedure.
Types of NCCI Edits
- Column 1/Column 2 Edits: The Column 2 code is bundled into the Column 1 code and generally cannot be billed separately.
- Modifier Indicator 0: No modifier will allow separate billing.
- Modifier Indicator 1: A modifier may allow separate billing if documentation supports distinct services.
Common Modifiers for Unbundling
- Modifier 25: Significant, separately identifiable E/M service
- Modifier 59: Distinct procedural service
- Modifier XE: Separate encounter
- Modifier XP: Separate practitioner
- Modifier XS: Separate structure
- Modifier XU: Unusual non-overlapping service
